    How to Exercise Your Rights

    Submit a Request

    Most rights can be exercised by submitting a written request. We provide online forms for your convenience, or you can download and mail paper forms.

    Response Time

    We will respond to most requests within 30 days. For access requests, we may have up to 60 days in certain circumstances.

    Fees

    We may charge a reasonable fee for copies of your health information, based on our costs for producing and mailing the copies.
