What this page is for
This page explains how to request an accounting of disclosures of your protected health information (PHI) held on the Elderwise platform. An accounting is a list of certain occasions when your information was shared. When Elderwise acts on behalf of a healthcare provider, we help that provider handle your request.
What you can request
You can request a list of disclosures of your PHI made in the six (6) years before the date of your request, or choose a shorter period. Where HIPAA applies, the accounting covers disclosures subject to the accounting requirement, including those made to or by the provider's business associates.
An accounting does not include every use or disclosure of your information. Exclusions include:
- Disclosures for treatment, payment and healthcare operations.
- Disclosures you authorised.
- Disclosures made to you.
- Disclosures incidental to an otherwise permitted or required use or disclosure.
- Other categories excluded by applicable law.

Who handles your request
If your care is coordinated through a healthcare provider using Elderwise, that provider is normally responsible for your accounting. You can contact the provider directly. If you contact us, we will forward your request to the provider or support its response when we act on its behalf.

How to submit
Email dpo@ajentik.ai with the subject "Accounting of disclosures request". To help us locate your records and direct your request, include:
- Your full name.
- Your date of birth.
- Your contact details so we can reply.
- The period you want the accounting to cover.
- The healthcare provider or organisation involved.
- Your preferred delivery format.
Identity verification
We verify your identity before releasing any information and may ask for additional documents. We will explain what is needed and how to provide it. We never ask for your passwords.
Timelines
We acknowledge requests within 5 business days.
Where HIPAA applies, the responsible provider, or Elderwise where responsible, will provide the accounting within 60 days of receiving your request. If more time is needed, one extension of up to 30 days is permitted. Before the initial 60-day period ends, you will receive written notice explaining the reason for the delay and the date by which the accounting will be provided.
Where Singapore's Personal Data Protection Act (PDPA) applies, we aim to respond within 30 days. If we need more time, we will let you know when you can expect a response.
Fees
Your first accounting request in any 12-month period is free. A reasonable, cost-based fee may apply to further requests in that period. You will be told about any fee in advance and can withdraw or modify your request to avoid or reduce the fee.
